avelumab + azacitidine is a combination therapy regimen investigated by the M.D. Anderson Cancer Center for the treatment of relapsed or refractory acute myeloid leukemia (AML). The regimen pairs avelumab, a human anti-PD-L1 IgG1 monoclonal antibody, with azacitidine, a hypomethylating agent. Avelumab functions as an immune checkpoint inhibitor by blocking the interaction between PD-L1 on tumor cells and PD-1 on T-cells, thereby restoring anti-tumor immune activity. Azacitidine, a pyrimidine nucleoside analog, incorporates into DNA and inhibits DNA methyltransferases, leading to hypomethylation and the reactivation of tumor suppressor genes, while also exerting direct cytotoxic effects on hematopoietic cells. In clinical studies, the combination was found to be well-tolerated but demonstrated limited clinical efficacy in the relapsed/refractory AML setting.
