Avelumab + mFOLFOX6 is an investigational combination therapy consisting of the monoclonal antibody avelumab (an anti-PD-L1 immune checkpoint inhibitor) and the chemotherapy regimen known as modified FOLFOX6 (mFOLFOX6). Avelumab blocks programmed death-ligand 1 (PD-L1), enhancing T-cell-mediated antitumor immune responses. The mFOLFOX6 regimen includes oxaliplatin, folinic acid (leucovorin), and fluorouracil (5-FU), which act as cytotoxic agents targeting rapidly dividing cancer cells. This combination is being studied primarily in gastrointestinal cancers such as metastatic colorectal cancer, locally advanced rectal cancer, and HER2-amplified gastric or esophageal adenocarcinomas. The rationale for combining immunotherapy with chemotherapy is to improve tumor response rates by both direct cytotoxic effects on tumor cells and modulation of the tumor microenvironment to enhance immune activity[1][2][5].
