Avelumab + paclitaxel + pelareorep is an investigational combination therapy for cancer, specifically studied in hormone receptor-positive, HER2-negative metastatic breast cancer. The regimen combines three agents with distinct mechanisms: - **Avelumab** is a human anti-PD-L1 monoclonal antibody (checkpoint inhibitor) that blocks the interaction between PD-L1 and PD-1, thereby enhancing T-cell mediated immune responses against tumor cells. - **Paclitaxel** is a microtubule-stabilizing chemotherapeutic agent that inhibits cell division by preventing microtubule depolymerization. - **Pelareorep** (Reolysin) is an intravenously delivered oncolytic reovirus that selectively infects and lyses tumor cells—especially those with activated Ras signaling—and induces local and systemic immune activation by promoting infiltration of tumor-infiltrating lymphocytes and expanding T-cell clones. The combination aims to leverage direct cytotoxicity (paclitaxel), immune checkpoint blockade (avelumab), and immunogenic viral oncolysis (pelareorep) to induce robust antitumor activity. In the phase 2 BRACELET-1 trial, this triplet was evaluated for safety and efficacy in metastatic breast cancer patients who were endocrine-refractory[6][5][1].
