AvidinOX + 177Lu-biotin is a pretargeted radiopharmaceutical therapy developed by Kiran Biotech for the treatment of inoperable liver metastases. The platform utilizes AvidinOX, a proprietary, chemically modified form of avidin that covalently binds to the extracellular matrix and cell surface proteins at the site of administration (e.g., via hepatic artery injection). Once anchored to the tumor tissue, AvidinOX acts as a high-affinity capture agent for subsequently administered biotinylated compounds. In this specific combination, lutetium-177 (177Lu), a beta-emitting radioisotope, is conjugated to biotin. The high-affinity interaction between avidin and biotin ensures that the radiation is localized to the tumor site, maximizing the therapeutic dose to cancer cells while minimizing systemic exposure and off-target toxicity. This approach is currently being evaluated in Phase 1 clinical trials.
