**Axalimogene filolisbac + durvalumab** is an experimental combination therapy for advanced, recurrent, or refractory human papillomavirus (HPV)-associated cancers, including cervical and head and neck cancers. **Axalimogene filolisbac (AXAL, ADXS11-001)** is a live attenuated *Listeria monocytogenes*-based immunotherapy engineered to secrete a fusion protein of truncated listeriolysin O and HPV-16 E7, activating both innate and adaptive immune responses, leading to tumor-specific T-cell activation and downregulation of tumor immune tolerance[2][4][6]. **Durvalumab (Imfinzi, MEDI4736)** is a human monoclonal antibody against PD-L1, blocking its interaction with PD-1 on T cells, thereby promoting anti-tumor immune responses[3][5]. The combination is being investigated to synergistically target HPV-associated tumors by stimulating immune response via two different mechanisms: one as a therapeutic vaccine stimulating HPV-specific immunity, and the other as a checkpoint inhibitor releasing immune inhibition. The regimen is under clinical evaluation but has experienced a clinical hold due to safety concerns[1][5].
