Overview

Axicabtagene ciloleucel + utomilumab is a combination immunotherapy consisting of axicabtagene ciloleucel, an autologous anti-CD19 chimeric antigen receptor (CAR) T-cell therapy, and utomilumab, a monoclonal antibody that acts as an agonist of the costimulatory receptor 4-1BB (CD137). This combination is being investigated in relapsed/refractory large B-cell lymphoma (LBCL) to enhance the efficacy of CAR T-cell therapy. Axicabtagene ciloleucel genetically modifies a patient's own T cells to recognize and destroy CD19-positive B cells, while utomilumab promotes T-cell proliferation and survival through 4-1BB agonism, potentially improving CAR T-cell expansion, persistence, and antitumor activity[3][5][1]. This dual costimulation approach aims to overcome the limited durability of response seen in some patients with CAR T-cell monotherapy.
