Azacitidine + APL-4098 is an investigational combination therapy being studied for the treatment of relapsed or refractory acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS), including MDS with excess blasts. Azacitidine is a demethylating agent that works by inhibiting DNA methylation, thereby promoting normal blood cell production and inducing death in abnormal bone marrow cells. It is approved for use in MDS and related disorders. APL-4098 is a highly selective and potent small molecule inhibitor of GCN2 (General Control Nonderepressible 2 kinase), developed as a novel therapeutic for liquid tumors such as AML and MDS. The combination aims to enhance antitumor activity through complementary mechanisms: epigenetic modulation by azacitidine and inhibition of stress response pathways via GCN2 blockade by APL-4098. The combination is currently being evaluated in Phase 1/2 clinical trials to assess safety, tolerability, pharmacokinetics, response rate, and duration of response[1][3][4][5][6].
