The combination of azacitidine and hematopoietic growth factors is an investigational therapeutic regimen evaluated for the treatment of myelodysplastic syndromes (MDS). This regimen consists of **azacitidine**, a nucleoside analogue and DNA methyltransferase inhibitor, paired with two biologics: **darbepoetin alfa**, an erythropoiesis-stimulating agent, and **filgrastim** (granulocyte colony-stimulating factor, G-CSF). Azacitidine works by incorporating into DNA and RNA, leading to hypomethylation and the restoration of normal growth control and differentiation in hematopoietic cells. Darbepoetin alfa and filgrastim are added to stimulate the production of red blood cells and neutrophils, respectively, aiming to improve hematologic responses and mitigate the cytopenias often associated with both the underlying disease and chemotherapy. This specific combination was notably evaluated in a Phase 2 clinical trial sponsored by Wake Forest University Health Sciences.
