Azacitidine + interferon is a combination therapy consisting of the hypomethylating agent azacitidine and an immunomodulatory cytokine, typically interferon-alpha (IFN-α). Azacitidine is a pyrimidine nucleoside analogue that inhibits DNA methyltransferase, leading to DNA hypomethylation and reactivation of silenced genes. It also incorporates into RNA and DNA, disrupting their metabolism and synthesis. Interferon-alpha exerts antitumor effects by modulating immune responses, inducing antiproliferative activity, promoting apoptosis in malignant cells, and enhancing immunogenic cell death. The combination has been investigated as pre-emptive therapy for patients with acute myeloid leukemia (AML) or myelodysplastic syndromes (MDS), particularly post-allogeneic stem cell transplantation with measurable residual disease (MRD) positivity. Clinical studies indicate this regimen can prevent disease progression with manageable toxicity profiles[1][7][5].
