Overview

Azacitidine + lenalidomide is a combination therapy used primarily in the treatment of higher-risk myelodysplastic syndromes (MDS), acute myeloid leukemia (AML), and chronic myelomonocytic leukemia (CMML). Azacitidine is a DNA methyltransferase inhibitor that works by incorporating into DNA and RNA, leading to hypomethylation of DNA and direct cytotoxicity to abnormal hematopoietic cells. Lenalidomide is an immunomodulatory agent derived from thalidomide; it exerts its effects by modulating cytokine production, enhancing T cell and natural killer cell activity, inhibiting angiogenesis, and promoting the degradation of specific transcription factors via cereblon-mediated ubiquitination. The combination leverages non-overlapping mechanisms—epigenetic modification by azacitidine and immune modulation plus direct anti-tumor activity by lenalidomide—to improve clinical outcomes in patients with high-risk hematologic malignancies[1][2][4][6].
