Azacitidine + sabatolimab is an investigational combination of azacitidine, a hypomethylating agent (HMA), and sabatolimab, a monoclonal antibody targeting TIM-3 (T-cell immunoglobulin and mucin-domain containing-3), developed for the treatment of higher-risk myelodysplastic syndromes (MDS) and chronic myelomonocytic leukemia (CMML). Azacitidine works by incorporating into DNA and RNA to induce hypomethylation and cytotoxicity in abnormal hematopoietic cells, while sabatolimab blocks TIM-3, modulating immune responses and aiming to enhance antitumor activity by inhibiting LSCs (leukemic stem cells) and reactivating immune effector cells. The combination is designed to provide durable clinical benefit for patients not eligible for intensive chemotherapy or stem cell transplant, with the rationale that sabatolimab may enhance the efficacy of azacitidine by overcoming immune resistance mechanisms[4][1].
