AZD1305 + digoxin is an experimental fixed-dose combination of AZD1305, a novel atrial-selective antiarrhythmic agent, and digoxin, a well-established cardiac glycoside. AZD1305 acts as a combined ion channel blocker targeting multiple cardiac ion channels, namely the rapid delayed rectifier potassium current, the inward sodium current (including both peak and late components), and the L-type calcium current, resulting in delayed repolarization and increased refractory period in cardiac myocytes, with atrial selectivity. Digoxin inhibits the Na+/K+-ATPase pump, leading to increased intracellular calcium and positive inotropic effects, and is used to treat heart failure and control ventricular rate in atrial fibrillation. The intent of the AZD1305 + digoxin combination is to assess potential pharmacokinetic or pharmacodynamic interactions when both agents are administered together, especially with respect to safety and rhythm control in atrial fibrillation. This combination has been studied in early-phase clinical trials only, mainly to evaluate pharmacokinetic interactions and not established as a marketed therapy.
