Overview

AZD5305 + trastuzumab deruxtecan is an investigational combination therapy currently being studied primarily for advanced solid tumors, including metastatic breast cancer and ovarian cancer. AZD5305 is a next-generation, highly selective oral PARP1 inhibitor designed to impair cancer cell DNA repair, thereby inducing synthetic lethality in tumor cells with DNA repair deficiencies. Trastuzumab deruxtecan (T-DXd, brand name Enhertu) is an antibody-drug conjugate (ADC) that targets HER2, delivering a cytotoxic topoisomerase I inhibitor directly to HER2-expressing cancer cells. This combination seeks to exploit the complementary mechanisms: AZD5305 potentially increases reliance on topoisomerase I–mediated pathways for DNA repair, increasing susceptibility to damage inflicted by T-DXd. Early data from phase I/II PETRA studies have shown promising signs of tumor response in heavily pretreated metastatic breast cancer, particularly triple-negative breast cancer[1][7].
