Overview

**Azelaic acid + doxycycline** is a non-branded combination therapy used primarily in dermatology for the treatment of mild-to-moderate and moderate-to-severe papulopustular rosacea. Azelaic acid is a topical dicarboxylic acid with anti-inflammatory, antimicrobial, and antioxidant properties. It inhibits tyrosinase, mitochondrial respiratory chain enzymes, DNA synthesis, and the cathelicidin pathway, reducing inflammation and skin lesions[4][3][8]. Doxycycline is a tetracycline-class oral antibiotic that also provides anti-inflammatory benefits at sub-antimicrobial dosing. Together, topical azelaic acid (typically 15% gel) and oral doxycycline (typically 40–100 mg/day) exhibit additive or synergistic effects, leading to improved lesion reduction and faster onset of action compared to monotherapy, without significant additional adverse effects. This combination is supported by clinical studies for rosacea and is sometimes used off-label for other inflammatory dermatological conditions[1][2][3][7][9].
