Azithromycin + amoxicillin is a combination of two antibiotics from different classes, sometimes used together in clinical practice for the empirical treatment of certain bacterial infections when the causative organism is not yet identified. Azithromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 23S rRNA of the 50S ribosomal subunit, thereby blocking transpeptidation and translocation steps essential for protein production. Amoxicillin is a beta-lactam (penicillin-type) antibiotic that interferes with bacterial cell wall synthesis by inhibiting the transpeptidase enzyme responsible for cross-linking peptidoglycan chains, leading to cell lysis and death. Both drugs are broad-spectrum antibiotics but have different mechanisms and spectra of activity. This combination may be considered in select cases such as severe pneumonia or mixed infections where coverage for both typical and atypical bacteria is desired; however, routine use together is generally unnecessary unless specifically indicated[6][8][4].
