Azithromycin + cefazolin is a combination of two antibiotics used primarily as perioperative prophylaxis to prevent postoperative infections, especially in women undergoing cesarean delivery. Azithromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, providing coverage against atypical and some Gram-negative organisms. Cefazolin is a first-generation cephalosporin antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, offering broad-spectrum activity against many Gram-positive bacteria and some Gram-negative bacteria. The combination has been shown in randomized controlled trials and clinical guidelines to significantly reduce the incidence of surgical site infections, endometritis, febrile morbidity, and hospital stay compared with cefazolin alone[1][3][5][7]. This regimen is particularly recommended for cesarean section prophylaxis where broader antimicrobial coverage is desired.
