azithromycin + dornase alfa (RhDNase) is a combination of two standard-of-care respiratory therapies used in the management of cystic fibrosis (CF). Azithromycin is a macrolide antibiotic that provides both antibacterial activity and anti-inflammatory effects, often used to reduce pulmonary exacerbations in CF patients colonized with Pseudomonas aeruginosa. Dornase alfa is a recombinant human deoxyribonuclease I (RhDNase) that selectively cleaves extracellular DNA in the mucus of CF patients, thereby reducing the viscosity of sputum and improving airflow. This specific combination is the subject of the MODUCO study, a population-based comparative effectiveness study led by Hospices Civils de Lyon, which investigates whether the introduction of highly effective CFTR modulators leads to the discontinuation or reduction of these traditional co-therapies and how such changes impact clinical outcomes like lung function.
