Overview

Azithromycin + lactulose is a combination pharmaceutical product developed by Avva Rus and marketed in Russia under the brand name Ecomed (Azithromycin Ecomed®). The product combines the macrolide-azalide antibiotic azithromycin with the prebiotic disaccharide lactulose. Azithromycin exerts its antibacterial effect by binding to the 50S subunit of the bacterial ribosome, thereby inhibiting protein synthesis and bacterial growth. Lactulose is included to mitigate the common side effect of antibiotic-associated dysbiosis; it acts as a prebiotic substrate that is fermented by saccharolytic commensal bacteria (e.g., Lactobacillus and Bifidobacterium), promoting their growth and maintaining gut microbial balance during antibiotic therapy. This combination is primarily indicated for pediatric use in the treatment of acute bacterial rhinosinusitis and otitis media.
