Azithromycin + loteprednol is a fixed-dose combination ophthalmic suspension containing azithromycin, a macrolide antibiotic, and loteprednol etabonate, a corticosteroid. This combination is primarily used for the treatment of ocular conditions involving both infection and inflammation, such as blepharitis, blepharoconjunctivitis (inflammatory conditions with bacterial involvement), trachoma (a chronic infectious eye disease), and infections associated with inflammation like bacterial conjunctivitis with coexistent ocular inflammation. Azithromycin acts by inhibiting bacterial protein synthesis through binding to the 50S ribosomal subunit of susceptible bacteria. Loteprednol exerts its anti-inflammatory effects mainly via modulation of the cytosolic glucocorticoid receptor at the genomic level; it upregulates anti-inflammatory proteins and represses proinflammatory proteins after binding to these receptors in target cells[1][6]. The combination aims to address both microbial eradication and inflammatory control in post-operative or infectious ocular diseases[1][2].
