Overview

Azithromycin + metronidazole is a combination of two antibiotics used together for the treatment of certain bacterial and protozoal infections. Azithromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, leading to inhibition of RNA-dependent protein synthesis. Metronidazole is a nitroimidazole antibiotic and antiprotozoal agent that works by disrupting DNA synthesis in susceptible anaerobic bacteria and protozoa through reduction of its nitro group, generating cytotoxic intermediates. This combination has been studied for use in pediatric Crohn’s disease (for induction or maintenance of remission), acute pelvic inflammatory disease (PID), and other mixed infections where both aerobic/mixed flora (targeted by azithromycin) and anaerobic/protozoal pathogens (targeted by metronidazole) are present[6][7][8]. The combined regimen may be more effective than monotherapy in certain clinical scenarios but carries an increased risk for adverse effects such as QT prolongation[1].
