This combination regimen consists of azithromycin, rifabutin (or rifampin), and ethambutol, and was specifically evaluated by The University of Texas Health Science Center at Tyler for the treatment of Mycobacterium avium complex (MAC) lung disease. Azithromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. Rifabutin and rifampin are rifamycins that inhibit DNA-dependent RNA polymerase, while ethambutol inhibits arabinosyltransferase, disrupting cell wall synthesis. This three-times-weekly multidrug regimen was studied in a Phase 4 open-label clinical trial (NCT00598962) to determine its safety and efficacy in HIV-negative patients with MAC lung disease, aiming to provide a simplified dosing schedule compared to daily administration.
