Azithromycin + scaling and root planing (SRP) is a combination therapeutic approach for the treatment of chronic periodontitis. This specific protocol, evaluated by the Ukrainian Medical Stomatological Academy, involves the oral administration of azithromycin (a macrolide antibiotic) as an adjunct to the standard mechanical debridement of SRP. Azithromycin is administered in a long-term course (500 mg daily for 7 days, followed by 500 mg weekly for 3 weeks). Beyond its primary antibacterial activity against periodontal pathogens, the treatment aims to leverage the immunomodulatory effects of azithromycin, specifically its ability to influence macrophage polarization by increasing the density of CD68+ and CD163+ macrophages in gingival tissues, thereby promoting a shift toward an anti-inflammatory environment.
