The aztreonam + gentamicin combination is an antibiotic regimen that combines two distinct antimicrobial agents with complementary mechanisms of action. This combination has been studied for its synergistic effects against various gram-negative bacterial infections, particularly those caused by Pseudomonas aeruginosa. ## Pharmacology and Mechanism of Action Aztreonam is a monobactam antibiotic that works by inhibiting bacterial cell wall synthesis[8]. It selectively targets gram-negative aerobic bacteria while having minimal activity against gram-positive bacteria[10]. Aztreonam binds to penicillin-binding proteins (PBPs) in susceptible bacteria, disrupting cell wall formation and leading to bacterial cell death[8]. Gentamicin is an aminoglycoside antibiotic that functions by binding to the 30S subunit of the bacterial ribosome, which interferes with protein synthesis[6]. This binding forces adenosines to maintain positions that lead to the acceptance of incorrect amino acid-tRNAs, resulting in the production of non-functional, mistranslated proteins that misfold and aggregate, eventually killing the bacterium[6]. When combined, aztreonam and gentamicin have demonstrated synergistic activity against many gram-negative bacteria, including: - Most strains of Pseudomonas aeruginosa - Many strains of Enterobacteriaceae - Other gram-negative aerobic bacilli[8][9] ## Clinical Applications The aztreonam + gentamicin combination has been studied for treating: 1. Lower respiratory tract infections due to Pseudomonas aeruginosa[1] 2. Enterobacter aerogenes endocarditis[2][5] 3. Infections in patients with cystic fibrosis[3] 4. Pseudomonas cepacia infections[3] In clinical studies, the combination has shown impressive efficacy: - In one multicenter study of lower respiratory tract infections due to P. aeruginosa, the clinical response rate was 84% (48/57 patients), including 47% complete cures and 37% partial responses[1] - The microbiological response rate in the same study was 69% (35/51 patients)[1] - The combination was synergistic in 72% of initial isolates from patients[1] ## Dosing Typical dosing regimens used in studies include: - Aztreonam: 2 g every 8 hours[1] - Gentamicin: 3 to 5 mg/kg per day in three equal doses[1] Alternative dosing used in animal studies: - Aztreonam: 60 mg/kg every 6 hours[2][5] - Gentamicin: 1.7 mg/kg every 8 hours[2][5] ## Pharmacokinetics When administered separately: - Aztreonam: - Peak serum level: approximately 135 μg/ml - Half-life: 0.7 hours (in animal studies)[2][5] - In humans with normal renal function, the serum half-life averages 1.7 hours (1.5 to 2)[8] - Serum protein binding: approximately 56%[8] - Gentamicin: - Peak serum level: approximately 7.4 μg/ml - Half-life: 1.3 hours (in animal studies)[2][5] Single-dose intravenous pharmacokinetic studies have not shown any significant interaction between aztreonam and concomitantly administered gentamicin[9]. ## Safety and Tolerability The aztreonam + gentamicin combination was generally well tolerated in clinical studies[1]. In one study, two patients were withdrawn due to rashes that emerged during treatment[1]. The combination may offer advantages over other antibiotic combinations with regard to toxicity[4]. This combination represents an important therapeutic option for serious gram-negative infections, particularly in cases where synergistic activity is desired to enhance efficacy or prevent the emergence of resistance.
