**Aztreonam + nacubactam** is an investigational intravenous antibacterial combination consisting of aztreonam, a monobactam β-lactam antibiotic, and nacubactam, a diazabicyclooctane β-lactamase inhibitor. Aztreonam primarily inhibits bacterial penicillin-binding protein 3, while nacubactam inhibits clinically important serine β-lactamases and directly binds penicillin-binding protein 2, producing a complementary cell-wall-targeting enhancer effect. Because aztreonam is stable to metallo-β-lactamase hydrolysis, nacubactam can protect it from co-produced serine β-lactamases, supporting activity against carbapenem-resistant Enterobacterales including metallo-β-lactamase-producing pathogens. The regimen is in Phase 3 development for complicated urinary tract infection, acute uncomplicated pyelonephritis, hospital-acquired bacterial pneumonia, ventilator-associated bacterial pneumonia, and complicated intra-abdominal infection caused by carbapenem-resistant Enterobacterales.
