Overview

This is a combination therapy consisting of intravesical Bacillus Calmette-Guérin (BCG), an attenuated live strain of Mycobacterium bovis used as an immunotherapy for non-muscle-invasive bladder cancer (NMIBC), and interferon alpha (IFN-α), a recombinant cytokine with immunomodulatory and antitumor properties. The rationale for combining these agents is based on their potential synergistic effects in enhancing the local immune response against bladder tumor cells. BCG stimulates a Th1-type immune response leading to cytokine release and recruitment of cytotoxic immune cells, while IFN-α further polarizes the immune response toward Th1 by upregulating IFN-γ, IL-12, TNF-α production and downregulating Th2 cytokines such as IL-10. This combination has been investigated primarily in patients who are unresponsive or intolerant to standard-dose BCG monotherapy or at high risk for recurrence after transurethral resection of bladder tumors[1][3][5]. Clinical studies have shown mixed results regarding efficacy compared to BCG alone; some suggest no significant reduction in recurrence but possible increased toxicity such as fever and constitutional symptoms[2]. The regimen is not universally approved but may be considered in select clinical scenarios.
