**Bacitracin zinc + hydrocortisone + polymyxin b sulfate + neomycin** is a fixed-dose ophthalmic ointment combining three topical anti-infectives with a corticosteroid for treatment of steroid-responsive inflammatory ocular conditions when bacterial infection is present or there is a risk of infection. Hydrocortisone suppresses ocular inflammation through glucocorticoid receptor-mediated anti-inflammatory effects, while neomycin, polymyxin B, and bacitracin provide complementary antibacterial coverage by inhibiting bacterial protein synthesis, disrupting bacterial cell membranes, and inhibiting bacterial cell wall synthesis, respectively. The product is used in inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe, including chronic anterior uveitis and corneal injury from chemical, radiation, or thermal burns or foreign-body penetration, and is marketed as a generic ophthalmic ointment by manufacturers including Bausch & Lomb.
