Baclofen + amitriptyline + ketamine, commonly referred to as BAK gel, is a compounded topical formulation designed for the treatment of neuropathic pain, particularly chemotherapy-induced peripheral neuropathy (CIPN). The formulation typically consists of baclofen (a GABA-B receptor agonist), amitriptyline hydrochloride (a tricyclic antidepressant that blocks sodium channels and inhibits monoamine reuptake), and ketamine hydrochloride (an NMDA receptor antagonist). These active ingredients are usually incorporated into a pluronic lecithin organogel (PLO) base to facilitate transdermal delivery while minimizing systemic absorption and associated side effects. The combination was evaluated in a randomized, double-blind, placebo-controlled Phase III clinical trial (NCT00516503) conducted by the Cancer and Leukemia Group B (CALGB). While the trial demonstrated trends toward improvement in sensory and motor symptoms of neuropathy, the results did not reach statistical significance compared to placebo.
