Overview

Balstilimab + ivermectin is an investigational combination therapy being studied for the treatment of metastatic triple-negative breast cancer (TNBC). Balstilimab is a monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, functioning as an immune checkpoint inhibitor to enhance T-cell mediated anti-tumor responses. Ivermectin, traditionally used as an antiparasitic agent, has demonstrated immunomodulatory and potential anticancer properties by increasing tumor infiltration of CD4+ and CD8+ T-cells and reducing myeloid-derived suppressor cells in preclinical models. The rationale for this combination is that ivermectin may "prime" tumors—turning immunologically "cold" tumors into "hot" ones—thereby enhancing the efficacy of immune checkpoint blockade with balstilimab. Clinical trials are ongoing to evaluate safety, optimal dosing, and preliminary efficacy in patients with metastatic TNBC[1][2][4][5][6][7].
