EBV-Specific CTLs + CD45 monoclonal antibody is an investigational combination immunotherapy developed for the treatment of recurrent or metastatic EBV-positive nasopharyngeal carcinoma (NPC). The regimen consists of autologous EBV-specific cytotoxic T-lymphocytes (CTLs), known as baltaleucel-T, and a pair of rat monoclonal antibodies (YTH 24/54) directed against the CD45 antigen. The anti-CD45 antibodies are administered prior to the CTL infusion to induce transient lymphodepletion, which has been shown to enhance the in vivo expansion of the T cells by increasing homeostatic cytokines like IL-15 and reducing regulatory T-cells. The CTLs are generated ex vivo by co-culturing the patient's peripheral blood mononuclear cells with irradiated EBV-transformed lymphoblastoid cell lines to target EBV-infected tumor cells. This approach was evaluated in a Phase I clinical trial (NCT00078546) by Baylor College of Medicine and the University Health Network.
