Overview

"Anti R-IL2 + Cyclosporine then Everolimus" is a combination immunosuppressive regimen used in the 'Switch' arm of a Phase 3 clinical trial (NCT01028092) sponsored by University Hospital, Brest. The regimen is designed for elderly renal transplant recipients receiving grafts from elderly donors (the 'old-for-old' strategy). It consists of induction therapy with the anti-IL-2 receptor monoclonal antibody basiliximab (Simulect), followed by maintenance therapy with mycophenolate mofetil (Cellcept), corticosteroids, and cyclosporine A (Neoral). The protocol involves an early switch from the calcineurin inhibitor cyclosporine to the mTOR inhibitor everolimus (Certican) to minimize CNI-related nephrotoxicity and improve long-term graft function.
