This therapeutic regimen is a de-novo calcineurin-inhibitor (CNI)-free immunosuppressive protocol designed for liver transplant recipients, particularly those presenting with impaired renal function at the time of surgery. The regimen, evaluated in the Phase II CILT trial led by Armin Goralczyk, consists of induction therapy with basiliximab (a chimeric monoclonal antibody targeting the IL-2 receptor alpha chain) followed by a maintenance combination of mycophenolate sodium (an IMPDH inhibitor), everolimus (an mTOR inhibitor), and prednisolone (a corticosteroid). By eliminating CNIs such as tacrolimus or cyclosporine, which are associated with significant nephrotoxicity, the regimen aims to preserve renal function while maintaining effective prophylaxis against acute organ rejection.
