Overview

**BC-819 + polyethylenimine + bacillus calmette-guerin vaccine** is a combination investigational therapy for non-muscle invasive bladder cancer. **BC-819** is a recombinant DNA plasmid encoding the diphtheria toxin A-subunit under the control of the tumor-specific H19 promoter, leading to selective toxin expression and tumor cell death in malignant urothelial cells. **Polyethylenimine (PEI)** is a cationic polymer used to enhance plasmid delivery into target cells—specifically here for efficient gene transfer into bladder cancer cells. **Bacillus Calmette-Guérin (BCG) vaccine** is a live attenuated strain of Mycobacterium bovis used as an immunotherapy standard of care for non-muscle-invasive bladder cancer, which activates the immune system against tumor cells. The proposed mechanism of this combination is direct cytotoxicity to cancer cells via toxin gene expression (BC-819) along with enhanced plasmid delivery (PEI), combined with immunostimulation and immune-mediated antitumor responses (BCG). This combination is designed for patients with intermediate- and high-risk non-muscle invasive bladder cancer, including BCG-unresponsive cases[1][2][4][5][8].
