BCG + mitomycin C is a combination intravesical therapy used primarily for the treatment of high-risk non-muscle-invasive bladder cancer (NMIBC). The regimen combines Bacillus Calmette-Guérin (BCG), an attenuated live strain of Mycobacterium bovis that acts as an immunotherapy by stimulating the immune system to attack bladder cancer cells, with mitomycin C, a cytotoxic antibiotic that inhibits DNA synthesis in rapidly dividing cells. This combination aims to enhance antitumor efficacy by leveraging both immune-mediated and direct cytotoxic mechanisms. Clinical trials have shown that sequential or alternating administration of these agents can improve disease-free survival and reduce recurrence rates compared to monotherapy. The combination may be administered using standard intravesical instillation or with electromotive drug administration (EMDA) to increase tissue penetration of mitomycin C[1][3][5][7].
