Overview

BCG (Bacillus Calmette-Guérin) is a live attenuated vaccine derived from the Mycobacterium bovis Danish strain 1331, traditionally used for tuberculosis prevention. Radboud University Medical Center investigated its potential to provide non-specific protection against SARS-CoV-2 in elderly populations through a mechanism known as "trained immunity." This process involves the epigenetic and metabolic reprogramming of innate immune cells, such as monocytes and natural killer cells, to enhance their response to subsequent infections. Despite evaluation in large Phase 4 randomized controlled trials (BCG-PRIME and BCG-CORONA-ELDERLY), the vaccine did not significantly reduce the incidence or severity of COVID-19 or other respiratory infections in the elderly population studied.
