Bcl-Xl_42 + CAF09b is a therapeutic cancer vaccine composed of a synthetic 42-amino acid peptide derived from the anti-apoptotic protein B-cell lymphoma-extra-large (Bcl-XL), formulated with the novel liposomal adjuvant CAF09b. The vaccine is designed to induce robust tumor-specific T cell responses by presenting the long peptide antigen to antigen-presenting cells (APCs), which then stimulate both CD4+ and CD8+ T cells. The adjuvant CAF09b consists of dimethyl-dioctadecyl ammonium (DDA), monomycoloyl glycerol (MMG), and polyinosinic-polycytidylic acid [poly(I:C)], a Toll-like receptor 3 agonist. This combination enhances immune activation and cross-presentation, favoring type I/CD8+ T cell responses. Developed for use in patients with hormone-sensitive prostate cancer, this vaccine has demonstrated safety and immunogenicity in phase I clinical trials[1][2][3].
