**BCMA + GPRC5D-directed CAR-T cell therapy** is a bispecific chimeric antigen receptor (CAR) T-cell therapy engineered to target both B-cell maturation antigen (**BCMA**) and G protein-coupled receptor family C group 5 member D (**GPRC5D**) on myeloma cells, addressing limitations of single-target BCMA CAR-T such as antigen escape due to BCMA-negative relapse. Autologous T cells are modified to express a bispecific CAR construct recognizing both targets, enabling enhanced tumor killing in relapsed/refractory multiple myeloma (**R/R MM**), including patients with extramedullary disease previously treated with BCMA therapies. Developed primarily in China by entities like Guangzhou Bio-Gene Technology, it has shown promising efficacy (e.g., VGPR in case reports) with manageable safety (e.g., grade 3 CRS controllable), and is under investigation in multiple phase 1/2 trials (NCT06068400, NCT07196124, NCT06644443).
