BCMA-CART-STriKE + IL-15 + anti-FAP + anti-CD16 is an experimental chimeric antigen receptor (CAR) T-cell therapy designed to treat multiple myeloma by targeting both the tumor cells and the immunosuppressive bone marrow microenvironment. Developed at the Mayo Clinic, this construct utilizes the "STriKE" (Secreted Tri-specific Killer Engager) platform, where BCMA-targeted CAR-T cells are engineered to secrete a tri-specific molecule composed of interleukin-15 (IL-15), an anti-fibroblast activation protein (FAP) moiety, and an anti-CD16 moiety. The BCMA CAR-T component directly targets and eliminates B-cell maturation antigen-expressing myeloma cells. Simultaneously, the secreted tri-specific molecule targets FAP-positive inhibitory cancer-associated fibroblasts (iCAFs) and recruits/activates CD16-positive innate immune cells (such as NK cells and macrophages) via IL-15 and CD16 binding to eliminate these fibroblasts. This dual approach aims to overcome resistance mediated by the tumor microenvironment and enhance the persistence and efficacy of the CAR-T therapy.
