BCNU + cisplatin is a combination chemotherapy regimen consisting of two cytotoxic agents, carmustine (also known as BCNU) and cisplatin. Carmustine is a nitrosourea alkylating agent that causes DNA cross-linking and inhibits DNA replication and RNA transcription, leading to cell death. Cisplatin is a platinum-based chemotherapeutic that forms DNA adducts, resulting in apoptosis through inhibition of DNA synthesis and function. This combination has been studied primarily for the treatment of malignant gliomas (including glioblastoma multiforme) in conjunction with radiotherapy[1][2]. The regimen has also been explored in other cancers such as advanced gastric carcinoma when combined with additional agents[4]. While the combination can be effective against certain tumors, it is associated with significant toxicities including hematologic toxicity (neutropenia, thrombocytopenia), nephrotoxicity, ototoxicity, nausea, and vomiting[1][2]. Its use has generally not shown clear survival advantages over other regimens like temozolomide for glioblastoma multiforme but remains an option where alternative therapies are limited or contraindicated[1][2].
