PDGF-BB + GEM21 refers to the combination of recombinant human platelet-derived growth factor BB (rhPDGF-BB, active ingredient: becaplermin) and a synthetic bone graft matrix composed of beta-tricalcium phosphate (β-TCP), commercially known as GEM 21S®. This combination is a bioactive grafting system designed for the treatment of periodontal and bone defects. rhPDGF-BB is a potent mitogen and chemotactic agent for cells of mesenchymal origin, including osteoblasts, periodontal ligament fibroblasts, and cementoblasts. It exerts its effects by binding to PDGF receptors (PDGFR-α and PDGFR-β), thereby stimulating the regeneration of alveolar bone, cementum, and the periodontal ligament. The β-TCP component serves as an osteoconductive scaffold that provides a framework for new bone formation while acting as a delivery vehicle for the growth factor. Research conducted at the University of Alabama at Birmingham (UAB) has further explored the release kinetics and efficacy of this combination in various regenerative contexts, including peri-implantitis.
