BPaMZ is an all-oral, fixed-duration combination regimen for tuberculosis (TB) consisting of four drugs: bedaquiline, pretomanid, moxifloxacin, and pyrazinamide. It is designed to treat both drug-susceptible and drug-resistant forms of pulmonary TB. The regimen has demonstrated high bactericidal activity and the potential to shorten treatment duration compared to standard therapies. Bedaquiline inhibits mycobacterial ATP synthase; pretomanid is a nitroimidazole that disrupts mycobacterial cell wall synthesis under aerobic conditions and releases reactive nitrogen species under anaerobic conditions; moxifloxacin is a fluoroquinolone that inhibits DNA gyrase; pyrazinamide disrupts membrane transport and energy production in Mycobacterium tuberculosis. BPaMZ has been evaluated in pivotal clinical trials such as SimpliciTB for its efficacy and safety in both drug-sensitive (DS-TB) and multidrug-resistant TB (MDR-TB)[1][2][3][7].
