BE-PEP (Bedaquiline-Enhanced Post-Exposure Prophylaxis) is an investigational combination drug regimen consisting of bedaquiline and rifampicin. Developed by the Institute of Tropical Medicine in Antwerp, Belgium, in collaboration with the Damien Foundation and Janssen Pharmaceuticals, it is designed for the prevention of leprosy in individuals who have been in close contact with leprosy patients. Bedaquiline is a diarylquinoline antibiotic that inhibits mycobacterial ATP synthase, while rifampicin is a rifamycin antibiotic that inhibits bacterial DNA-dependent RNA polymerase. The regimen is currently being evaluated in the Phase 3 BE-PEOPLE trial in the Comoros Islands to determine its efficacy compared to the standard single-dose rifampicin (SDR-PEP) protocol.
