unconjugated belantamab + pomalidomide + dexamethasone + Blenrep is an investigational combination regimen developed by GSK for the treatment of relapsed/refractory multiple myeloma. This four-drug regimen combines two anti-BCMA (B-cell maturation antigen) agents—the unconjugated (naked) monoclonal antibody belantamab and the antibody-drug conjugate (ADC) belantamab mafodotin (Blenrep)—with the immunomodulatory agent pomalidomide and the corticosteroid dexamethasone. The rationale behind this combination is to enhance anti-myeloma activity by utilizing both the direct cytotoxicity of the ADC's auristatin F payload and the immune-mediated effects of the naked antibody and pomalidomide. The regimen is currently being evaluated in the DREAMM-5 platform trial (NCT04091126), specifically in Part 3, to determine if the addition of the unconjugated antibody to the standard-of-care backbone can improve clinical outcomes in patients who have received prior therapies.
