**Belantamab mafodotin + feladilimab** is an investigational combination therapy for relapsed/refractory multiple myeloma. Belantamab mafodotin is an antibody-drug conjugate targeting B-cell maturation antigen (BCMA), delivering the cytotoxin monomethyl auristatin F (MMAF) into BCMA-expressing myeloma cells to induce apoptosis. Feladilimab is a monoclonal antibody targeting the Inducible T-cell COStimulator (ICOS), a costimulatory immune receptor, and acts as an agonist to enhance T-cell activation. This combination aims to exploit both direct anti-tumor cytotoxicity (via the ADC) and immune-mediated anti-myeloma activity (via ICOS agonism)[5]. The agents are being studied together in platform trials such as DREAMM-5, developed and sponsored by GSK, for patients with relapsed/refractory multiple myeloma after prior therapies[5].
