A combination therapy under investigation for relapsed/refractory multiple myeloma, consisting of belantamab mafodotin (an antibody-drug conjugate targeting B-cell maturation antigen (BCMA)) and nirogacestat (an oral, small molecule gamma-secretase inhibitor). Belantamab mafodotin blocks and kills BCMA-expressing myeloma cells through antibody-dependent mechanisms and delivery of a cytotoxic agent. Nirogacestat, by inhibiting gamma-secretase, increases membrane BCMA and reduces soluble BCMA, enhancing the effect of anti-BCMA therapies. This combination is being evaluated for synergistic anti-myeloma activity and reduction of high-grade ocular toxicity seen with belantamab mafodotin monotherapy. The combination is being studied primarily in patients with relapsed/refractory multiple myeloma who have had multiple prior lines of therapy[1][2][3][6].
