Belapectin + ipilimumab is a combination immunotherapy regimen being investigated for the treatment of metastatic melanoma. Belapectin (formerly GR-MD-02) is a complex carbohydrate polymer derived from citrus pectin that acts as an inhibitor of galectin-3, a protein often overexpressed in the tumor microenvironment that facilitates immune evasion and tumor progression. Ipilimumab is a recombinant human monoclonal antibody that blocks Cytotoxic T-Lymphocyte Antigen-4 (CTLA-4), a checkpoint molecule that inhibits T-cell activity. The combination therapy, developed through a collaboration between Galectin Therapeutics and Providence Health & Services, aims to enhance the anti-tumor immune response by simultaneously blocking CTLA-4 and inhibiting the immunosuppressive effects of galectin-3, thereby promoting the proliferation and activation of CD8+ T cells within the tumor milieu.
