Overview

Belimumab + cyclophosphamide is a combination therapy used primarily in the treatment of moderate-to-severe systemic lupus erythematosus (SLE), including neuropsychiatric manifestations and lupus nephritis. Belimumab is a human monoclonal antibody that inhibits B-cell activating factor (BAFF, also known as BLyS), thereby reducing the survival and differentiation of autoreactive B cells. Cyclophosphamide is an alkylating agent that suppresses immune cell proliferation by cross-linking DNA, leading to cell death, particularly in rapidly dividing lymphocytes. The combination aims to maximize disease control while minimizing adverse effects associated with higher doses of cyclophosphamide. Clinical studies have shown that this regimen can significantly reduce SLE disease activity scores, restore B/T cell balance, and lower infection rates compared to conventional treatments[1][2]. This strategy proposes a potential treatment approach for SLE patients who are refractory or intolerant to standard therapies.
