Belimumab + obinutuzumab is a combination of two monoclonal antibodies, each targeting different aspects of B cell biology. Belimumab is a fully human monoclonal antibody that binds to and inhibits B-lymphocyte stimulator protein (BLyS/BAFF), reducing the survival and differentiation of autoreactive B cells implicated in systemic lupus erythematosus (SLE) and lupus nephritis. Obinutuzumab is a glycoengineered, type II anti-CD20 monoclonal antibody that induces potent depletion of CD20+ B cells through enhanced antibody-dependent cellular cytotoxicity and direct cell death mechanisms. Both agents are used as immunotherapies for autoimmune diseases, particularly SLE and lupus nephritis, with evidence supporting their individual efficacy in these indications[4][6][7][9]. The rationale for combining these drugs lies in their complementary mechanisms—belimumab reduces the generation/survival of new autoreactive B cells while obinutuzumab depletes mature CD20+ B cells—potentially offering synergistic benefit in refractory or severe cases.
