Belimumab + voclosporin is a combination therapy consisting of two distinct immunomodulatory agents used in the management of lupus nephritis (LN), a severe manifestation of systemic lupus erythematosus (SLE). Belimumab is a fully human monoclonal antibody that targets and neutralizes B lymphocyte stimulator protein (BLyS, also known as BAFF), thereby inhibiting the survival and differentiation of autoreactive B cells implicated in SLE pathogenesis. Voclosporin is an oral calcineurin inhibitor that suppresses T cell activation by inhibiting calcineurin, leading to reduced cytokine production; it also stabilizes podocytes in the kidney, reducing proteinuria. Both drugs are approved individually for LN but have been used together off-label in clinical practice to achieve better disease control and facilitate steroid tapering. The combination has shown promise for safety and efficacy over 8–14 months in small case series, but formal clinical trials are needed to establish its role as standard therapy[1][7][9].
