Belrestotug + dostarlimab + chemotherapy is a discontinued combination immunotherapy regimen previously studied for first-line treatment of advanced, PD-L1 high non-small cell lung cancer (NSCLC). Belrestotug is a human IgG1 monoclonal antibody targeting TIGIT (T cell immunoreceptor with Ig and ITIM domains), designed to enhance anti-tumor immunity by disrupting TIGIT-mediated immunosuppression. Dostarlimab (Jemperli) is an anti–PD-1 monoclonal antibody that facilitates immune activation against tumors. The regimen was intended to synergistically block two key immune checkpoint pathways (TIGIT and PD-1) alongside chemotherapy, aiming to improve response rates and durable benefit. Phase 2 trials (GALAXIES Lung-201) showed increased response rates but failed to improve progression-free survival, leading to global termination of the program due to lack of efficacy.
