Bemcentinib + pembrolizumab is a combination therapy under clinical investigation for the treatment of advanced or metastatic non-small cell lung cancer (NSCLC), particularly in patients who have progressed after platinum-based chemotherapy and/or immune checkpoint inhibitor (ICI) therapy. Bemcentinib is a first-in-class, oral, highly selective AXL receptor tyrosine kinase inhibitor that targets mechanisms of resistance to immunotherapy by inhibiting AXL signaling, which is implicated in tumor immune evasion and epithelial-to-mesenchymal transition. Pembrolizumab is a monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor on T cells, thereby enhancing anti-tumor immune responses. The combination aims to reverse acquired resistance to checkpoint inhibition by targeting both tumor-intrinsic and microenvironmental factors. Clinical trials have shown promising efficacy and tolerability for this regimen in NSCLC patients with specific genetic mutations such as STK11, KEAP1, SMARCA4, and KRAS[1][2][3][6][8].
